Farley to Address Texas Chemical Council on Common Themes from Major Accidents Copy

On June 7, 2022, Partner Mark Farley will present at the Texas Chemical Council's 34th Annual Environmental Health and Safety Seminar on "Common Process Safety Issues from Major Industrial Accidents." Farley has worked with clients on the response to and investigation of some of the largest industrial incidents in the United States.
The specific facts and circumstances surrounding low frequency, high consequence process safety incidents are always different. These events often share similar contributing factors, however, including lack of hazard awareness, barriers to communication, and a false sense of confidence. Farley's presentation will consider five well-known accidents and discuss how similar process safety system failures contributed to the catastrophes. It also will provide practical advice on how organizations can identify these common deficiencies.
